We appreciate your participation and dedication to quality improvement and the Texas Rising Star certification program. 

Your Mentor will be in touch in the next few days to schedule a result meeting to go over the assessment results. Meanwhile, please see attached your facility assessment report from your recent assessment. Review, sign, date and submit back to me to acknowledge receipt of the assessment results.
