How to Screen Providers

 Screenings of ELPs are conducted by Assessors:  

  • Prior to any Assessment  

  • Quarterly (providers are screened during the first week of the last month of each quarter [March, June, September, December]. Re-screenings will occur at the end of each provider’s impact period.  

Certified Assessors will be assigned an assessment caseload and quarterly screening caseload. It is the logistics team’s responsibility to assign assessments and screen caseloads.
