Quality Assurance:  

Monthly data checks and data error reporting  

Assessment and screening data will be monitored monthly by the logistics manager and assessment managers using a burndown system to track the pace of assessor assignments, program assessments, ratings recorded in the Texas Rising Star assessment tool and in Engage, reliability routines, and data checks to help identify any discrepancies in data entry and corrections needed.  Correction will be included in the monthly assessment reports. The reports will be shared with boards and TWC.  

Documentation Retention: 

According to UTHealth Houston documentation retention policy, CAE documentation will be retained for 7 years. Assessment documentation includes Note-taking forms, FARF or any other paper form completed for assessments or screenings conducted. 

Assessors:  

Note-taking forms and Facility Assessment Record Forms or Classroom Assessment Record Forms used to conduct an assessment will be stored electronically. Assessor staff working remotely will be provided with the necessary packaging and postage to ship the completed forms every month by the 5th of the month, to the Operations Center Building located at 1851 Crosspoint Ave. Ste 2110, Houston, TX. 77054 

Assessors located in Houston and surrounding areas will drop off the note-taking forms and FARF with the Research Assistant staff, who will file the assessment documents. 

Logistics Team (RC’s, RA’s):

The documents shipped by the Assessors will be saved by the Research Assistant in a file folder and labeled with the school's name. All documentation related to the same school will be saved in the same file folder. If the assessment documentation is needed, it can be retrieved and reviewed.  

Assessment documents turned in by Assessors working remotely will also be filed by the Research Assistant.
