Assessors will work remotely across the State of Texas based on their home location. Assessors will print the note-taking forms, FARF, Visit Reports, and any other documents needed to conduct the assessment. Assessors will be provided with a printer, toner and printing paper. 

Rescheduling Assessments, ELP requested: 

If an assessment must be re-scheduled due to ELP refusing an assessment, the logistics team will re-schedule assessments as follows: 

  • Initial Assessment: Will be re-scheduled based on assessor availability – tentatively will be scheduled for the following month. 

  • Annual Monitoring: scheduled 4 – 6 weeks within the day of refusal 

  • Star Level Evaluation: must be re-scheduled and completed within the allowed timeframe – refer to the star level evaluation section. 

  • Category Reassessment: must be re-scheduled and completed within the allowed timeframe – refer to the category assessment section. 

  • Reconsideration: must be scheduled and completed within the allowed timeframe – refer to the reconsideration section. 

  • Facility Changes: must be rescheduled and completed within the allowed timeframe – refer to the facility changes section. 

  • Recertification: Must be re-scheduled and completed  

Rescheduling Assessments due to Assessor unavailability: 

In the event an Assessor notifies the logistics team and their manager that they will be unable to work due to unforeseen circumstances, the logistics team will reach out to the Assessors that are on-call that week (determine location and proximity). If more than one Assessor was assigned to complete the assessment, the logistics team will determine if the team of assessors can still conduct that assessment and re-assign a team lead for the assessment. 

Notifying Assessors of the Re-scheduling of an Assessment: 

The logistics team will update the scheduling calendar and re-submit the assignment.  

Assessors will use their cell phones (Teams App or text) to communicate with the logistics team as needed regarding the re-scheduling of assessments. 

Leave Request: 

Assessor staff will submit a leave request to their manager for approval via UTHealth Houston Self-Service People Soft  

Additionally, they will create an Outlook Calendar appointment and submit it to their manager and the CLI calendar, GM. The logistics team will utilize this information to schedule assessments accordingly.  The Assessor that is on leave will not be scheduled to conduct an assessment that day.
