The Team Lead Assessor will discuss the overall scoring, and other items, with the Mentor. If eligible, category reassessment: must be requested within 30 days of the ELP’s last full assessment. The assessor will assess only up to two of the lowest categories as requested by the ELP. If staff or director changes have occurred since the full assessment, Category 1 will also be reevaluated. This requirement will not count as one of the two allowable categories for reevaluation. This type of assessment does not guarantee the star level increase. The ELP would have access to Mentor support prior to the assessment being conducted.

The Assessment results for _____________________________ (include name of ELP and License #) have been finalized. I want to schedule a debrief meeting to review the assessment results with you. I am available on______________ between _________ and __________. Please let me know what works best and I will send you a virtual meeting invite.
Please note that the assessment reports will be emailed to the ELP in advance of your result meeting with the director. I will include you in this email.
