The Team Lead Assessor will email a copy of the facility and classroom result reports for the Director to review. Request the Director’s signature on the first page of the facility assessment report to be returned within 2 business days. The signed copy will be uploaded in Engage by creating a general status event log along with any visit reports and the the tracking form.
All local Assessors will drop off scan completed FARF, CARF (if used) and Observation Notes (note-taking forms) in the assigned bin for Research Assistants to file as needed. Assessors working remotely will ship their forms monthly to the designated location. and save to specified shared location file by the 4th of the month following the assessments.
