The Team Lead Assessor will email a copy of the facility and classroom result reports for the Director to review. Request the Director’s signature on the first page of the facility assessment report to be returned within 2 business days. The signed copy will be uploaded in Engage by creating a general status event log along with any visit reports and the the tracking form. 

 All local Assessors will drop off completed FARF, CARF (if used) and Observation Notes (note-taking forms) in the assigned bin for Research Assistants to file as needed. Assessors working remotely will ship their forms monthly to the designated location.
